Huntington Memorial Hospital, Senior Care Network Department, Pasadena, California                     1998-2009

Care Coordinator

Managed the care of 480 clients annually who were chronically mentally ill, developmentally disabled, frail/elder and at-risk for early institutionalization. This was an Integrated Case Management Program that was funded by the Older Americans Act, LINKAGES FUNDING, Disabled Parking Funding and the Title IIIE Care Giver Program. Assisted all clients in determining eligibility for public benefits. Provided case management services for the Multipurpose Senior Service Program.

  • Conducted psychosocial assessment for more than 640 clients at their home over ten years, including developing a plan of care and monitoring subsequent interventions on a monthly basis.

  • Prepared assessments, administered diagnostic tests, helped as needed on the senior hotline and provided caregivers with resources to prevent caregiver burnout.

  • Empowered clients and caregivers with decision-making as well as familial and caregiver conflicts.

  • Educated all clients and their family members on long-term care options and advance directives.

  • Prevented suicides by providing emotional support and concrete assistance during times of crisis.

  • Obtained funding from the Change of Life Grant for six clients over the past five years.

  • Worked closely with the Adult Protective Services, Law Enforcement, Office of the Public Guardian and Code Enforcement to ensure safety of all clients.

  • Presented difficult elder abuse cases to the Elder Abuse Task Force members of the East San Gabriel Valley multidisciplinary team for action.

  • Collaborated with the Elder Abuse Forensic team members for the protection, as needed, on cases regarding real estate fraud and conservatorship.

  • Coached the staff with advice and resources once a month at the Community Senior Services Center in Claremont.

  • Provided outreach consisting of help to seniors for resources once a month at Walnut Senior Center in the afternoons. Helped more than 150 seniors.

  • Provided training to other integrated care management agencies on how to administer and utilize the assessment tools for the Caregiver Title IIIE program.

  • Supervised Master of Social Work students in delivering case management services.

  • Intervened and prevented an elderly client from losing her homes and property to her children.

  • Participated in utilization reviews for more than 400 client charts on a quarterly basis.

Community and Senior Services of Los Angeles County, Adult Protective Services, Los Angeles, California 1997-1998

Spanish Speaking Social Worker (temporary position)

Investigated reports of alleged verbal, physical, financial abuse and neglect perpetrated on individuals 18 years old and older for the County of Los Angeles.

  • Assessed situations where clients may have been gravely disabled or were at eminent risk of serious injury or death.

  • Provided immediate information, interventions and referrals to community resources, including law enforcement, banks, offices of the public guardian and the district attorneys.

  • Prepared documentation for each case and monthly summaries concerning case status.

Bilingual Case Manager

Provided case management services to individuals living with HIV-AIDS for financial, housing, dental, medical, psychological, public benefits, legal and emotional support for the largest HIV-AIDS support agency in Los Angeles County.

  • Processed 20 new cases a month with ongoing support for more than 40 clients at a time.

  • Developed and implemented individual service plans tailored to the needs of each client.

  • Monitored case progress on a monthly basis, documenting each case daily.

  • Provided crisis intervention as needed for more than 200 clients.

  • Performed backup duties for clients when counselors were absent.

  • Responded to incoming calls for the intake department to get clients started with agency help.

East Valley Community Health Center, West Covina, California                                                          1994-1995

Health Educator and HIV Pre- and Post-Test Counselor

Educated teen-mothers at special high schools on HIV and STD prevention for a community health center covering the East San Gabriel Valley.

  • Made 64 presentations to more than 2,000 teen-mothers in high schools and for “English as a Second Language” programs.

  • Provided pre- and post-test HIV counseling for 390 patients coming into the Center for testing.

  • Created, wrote and delivered a complete curriculum on HIV prevention.

  • Provided limited case management for 20 clients.

Travelers Aid – Teen Canteen, Los Angeles, California                                                                               1994

Bilingual Case Worker

Provided case management services for runaway and homeless youths ages 14 to 24 at Los Angeles’ main drop-in center. The canteen provided bathing, laundry, lockers, food, counseling, case management, HIV prevention and job placement services.

  • Processed 8,660 youths over an eight-month period.  

  • Case managed 200 clients that wanted referrals for housing, financial support and work.

  • Provided advice on eligibility for extended benefits for all clients.
